PARENT/GUARDIAN RELEASE for Museum Teen Volunteers Ages 14 to 17:
The San Antonio Museum of Art is pleased to receive your child’s Museum Teen Volunteer
application. If accepted, your child will be assigned to assist with First Sundays for Families
programming. In signing this form, you grant permission for your child (please enter full name and birth date below) to participate fully in the museum's Museum Teen Volunteer program.
By agreeing below, you acknowledge that communication regarding the Museum Teen Volunteer Program will be between museum staff and your teen. You understand that your teen must have reliable transportation and that Museum teen volunteers who exceed three unscheduled (3) absences will be asked to leave the program. While the museum will take all reasonable steps to provide for the safety of your child, the museum and its employees and
agents shall not be liable for any injury, damage or harm that might result during the course of any volunteer activity. In permitting your teen to participate, you agree that such responsibility will remain with and is assumed by you, as the parent or guardian of your child. Should any claim be asserted as a result of the acts of your teen while participating as a volunteer or should your teen assert any claim against the museum, its employees or agents, you agree to
reimburse, indemnify, and hold the San Antonio Museum of Art harmless from any such claim, including attorneys’ fees. -- No answer -- Yes No
If a medical emergency arises, the museum will attempt to contact you or the person designated below. However, if the museum is unable to contact you immediately, you authorize the museum or its employees or agents to request and authorize, at your expense, emergency medical treatment.
